BAC Forms and Policies

This page is dedicated to providing a resource for forms, policies, and procedures.

Forms

• Athlete Code of Conduct - BAC Code of Conduct, this must be signed by the athlete and parent each year.
• BAC By-Laws - The By-Laws governing the BAC Board.
• Parent Code of Conduct - The BAC Code of Conduct for parents.  This document describes the BAC Board and Coaching Staff's expectations of the parents.
• Quarterly Dues Payment Coupons - This form needs to be filled out and mailed with a check to the treasurer each quarter.
• Registration Form - PDF  - This printable form needs to be filled out annually.
• Registration Form - Word  - This form needs to be filled out annually.  This has been created as an MS Word form - you can electronically edit the information, but you still must submit the information in paper form to BAC.
